Effectiveness of Bubble CPAP for Treatment of Children Aged 1-59 Months with Severe Pneumonia and Hypoxemia in Ethiopia: A Pragmatic Cluster Randomized Controlled Trial

In this article, authors share their prospective randomized clinical trial comparing use of bubble CPAP versus oxygen via low flow nasal cannula (WHO standard of care) in infants less than 5 years old hospitalized with severe pneumonia and hypoxemia in low resource settings. The design of their study is a cluster randomized controlled trial where six district hospitals are randomized to bubble CPAP and six to standard WHO low-flow oxygen therapy. The total sample size is 620 per arm. Currently, recruitment of the patients is ongoing. The primary outcome is treatment failure and main secondary outcome is death. This study will provide data on the effectiveness of bubble CPAP in treating childhood severe pneumonia and hypoxemia in a real-world setting. The Bubble CPAP Institute is an interactive educational platform aiming to make the knowledge and skill set of Bubble CPAP accessible to practitioners who care for neonates and young infants across the globe. This platform will primarily focus on the know-how and will offer educational modules on issues like when to consider bCPAP in managing infants with respiratory distress, how to assemble the bCPAP circuits, how to apply the nasal interface and how to troubleshoot bedside issues.
